The only concern I have is the strength of portable slot; it's a must take if you have car battery because of how powerful it is to grant yourself marginally better resources over simple coins. That said though, it made Portable Slot actually useful lol and it's another show of how creative this dev apparently is.

eternal d6: reduced chance to despawn
Spindown: turns into spinup
D6: turns the item into a type tainted isaac item
maybe that could of just been treated like the reverse Devil card and grant a temporary Seraphim familiar that shoots those sacred heart tears for the room?

Car Battery + The D6 could work like Soul of Isaac. It'd make items cycle the old item and a new rerolled item.
Some thoughts:
* That random rocket when you use Dr's remote sound like something that could easily cause bad damage. It should just fire a second missile.
* The Bible effect is probably too situation. It should spawn a permacharmed enemy if it can't convert any. Same for the book of the dead.
* Telepathy for dummies could give homing to your faliliars.
P.S. I'm not familiar with the mod API. I don't know if what I am suggesting is
actually possible.
Mega Blast: Upgraded from tier 4 -> 5 brimstone visually, and mechanically the multiplier goes from 5 -> 6.
Black hole: instantly hits all enemies in the room for 36 damage (the same as black hole does normally)
Smelter: 50% chance to make the trinket spawn on the floor after being absorbed.
Telepathy for dummies: permanently destroy the book and the car battery, gain spoon bender.
R key: rather than going back to the beginning, restart the current floor and spawn 1 boss item and treasure item for each floor that would have been reset.
Forget me now: you go backwards one floor (I.E stage 4 -> 3)
Flush: Spawn two dips for each enemy killed.
Flip: [T.laz] heal half a heart if going to laz, half a soul heart when going to dead laz. For other characters reroll the item before flipping (the item you swapped away from).
Friendly ball: when releasing, spawn the enemy twice.
Magic Fingers: 10 -> 20 flat damage.
Shoop da woop: Now a giga brimstone (tier 5), the same one beast uses. 2 -> 3x multiplier.
