I wasn't using any custom icons at the time of the recording, but I do currently use these higher quality HUD icons.

I finally figured out my missing audio issue, too many weapon script mods.

The voicelines will still play, they just won't be hearable.

For some reason since I installed this, the voicelines from the l4d2 rescues don't have any audio anymore.

You can turn down the volume if need be. The saferoom music is already adjusted not be too loud, unless everyone else thinks it's too loud as well.

I like the music change but could you tone it down a little change only the music when closing the saferoom door

That's how it is in the original mod. Nothing regarding volume has changed.
