What is happening to you is a conflict with another addon that also modifies the antlion model.
write the command ''whereis models/antlion.mdl" in the console
The command should give you the path of NPCVariants.

It is difficult to use both at the same time, although not impossible. However, this mod focuses on the variety of skins, not on creating small variations on the same skin, I think you should decide which of the 2 to use
The "combine_soldiers_melee_pack" addon you have installed interferes with the loading of the NPC Variants model. Uninstall one of the 2 addons, or the conflict will continue

try typing this command in your console, and see where the game is loading the model file for the combine soldiers:
