It is also recommended to uninstall other campaign addons since their files can conflict and cause instability.
It must be a problem with either your GMod itself or your Half-Life 2 games. Try verifying them all.
Check for any model addons you have and remember, this is only tested in Sandbox gamemode.

Addons that change the player size could be the problem, but I really can't pinpoint the cause of these kinds of problems.
Yes, unless given by the map at the start.
GMod doesn't feature checkpoints so if you respawn, you start all the way back to the beginning and activated map triggers stay activated.
It is recommended to either turn on god/buddha mode so there's no need to worry about death. Or go the extra mile and noclip from the beginning to where you died.
I recommend verifying those games' files as well as GMod's.

Where can i find it?
The only minor complaint I have is the toxic zone with the poison headcrabs, it'll instantly kill you if you get hit by a headcrab while in the gas.
Overall, 9/10, really really well done mate
Use Sandbox as that is the only gamemode I've tested for this.
