Based on recent trends, it's unlikely that this item would be restricted considering how relatively minimal it is ("Goblineer" is more extreme than this and can be worn year-round), but it's ultimately up to Valve to decide.

@setry you raise a solid point about the goblineer. its proof that valve does not follow its guidelines about tf2’s style as strongly as it used to, but at that point i would suggest that its up to workshop creators to set standards for the sake of the game looking neat and consistent. if this hat was added without the halloween restriction then it would look out of place with most of tf2. even items like the goblineer only occasionally slip by and get added without the restriction. unrestricted cosmetics that noticeably alter facial features generally conflict with character design and are uncommon in tf2

youve come up with a very good item here, but it wouldnt sit right with me if it was able to be worn all year round.

I can't agree with this at all
How can it confuse other players?
First of all the face is a very small detail compared to the body and people mostly recognize classes by their clothes/animation/body structure.
Second of all, goblineer isn't restricted and it gives engie a huge nose and ears. That item has been in the game for a year or 2 now and everything seems to be just fine.

The level three chin changes much more than these so and it's not restricted. Seems perfectly reasonable to me that this wouldn't be either
