hold C to open the context menu. click on the playermodels tab. select the "Engineer" playermodel. (or any other of my tf2 playermodels) and then click on the bodygroups tab. and from there pick any hat or misc you like!

It’s with bodygroups. So addons like ragmod will keep the hats on
