There is an entity that could cause you to hallucinate this in the backrooms, But that only applies to the original version of Level 0, The current version as anyone who has been on the The Backrooms Wikidot will know has no entities and no wanderers, Although you may still hallucinate this without any entity due to you being trapped and probably getting something contracted from the moldy floors.
