There's three different gore mods. You'll have to say who's gore mod you're using.
Lordaloa's doesn't work with any multiskin because his gore mod requires code to hide dead bodies so that the gore ragdoll functions like how he wants it to. That skin replacement code uses the same method to randomize character skins.

I just playtested with LeMieux's gore mod and that works. Lem's version uses bone scaling to hide limbs and attaches accessories to where the limbs would be to make the mangled limbs.
