It's not a model replacement. If custom model has face bones, then it works to some extent depending on how many effort did the author made on rigging.

Oddly enough, I actually remember when this game was first coming out there was a lot of concern about the sheer level of violence/gore that was in the game. Nowadays we take it for granted, but back in '09 there really weren't many (if any) games that portrayed gore in such a detailed and realistic manner. It doesn't really surprise me that they cut these facial animations out, just like they cut out several lines of explicit dialogue that only recently got restored with the Last Stand update. As things stood, I'm pretty sure that the final product barely skirted by with an M rating by the ESRB, and even then the game still got banned/censored in a lot of other places.

i guess to increase the game rating you could add amplified gore to special infected or addsome agonizing death scream for zombies for when they are set on fire for example, but yeah i guess that works too.
