Maybe I will look into that whenever I get back into working on this mod, but I have my doubts on that being any time soon.

Maybe try resetting the config and see if that magically fixes it?
Try disabling every mod except for MCM and FPP and see if it still breaks then.
Personally I use "MCM -Pure" and it seems to save the controls and all other settings for me just fine. You can find an easy link to it anywhere in the description MCM is mentioned.
Support for MCM Pure has already been implemented in the latest update, but y'all let me know if there is anything else in need of fixing or if settings don't save correctly.
As a temporary fix you could set the mod to "Push All" or "Phase All" mode.
Short summary of how the character works, picking up items such as coins, bombs or keys charge the meter on your ability, of which spawn an item. But the problem is that upon reaching the max amount of any pickup then trying to grab more will result your ability to charge anyway, basically giving you infinite items.
Note: As with any part of this mod, you can hold the drop key to interact with things like normal, in addition if you have MCM you can turn the feature completely off or change what coins it will effect.
Sorry if this has caused a inconvenience. Hopefully I have cleared things up, do feel free to give feedback on the feature if it annoys you.

As for why it only effects you in the custom challenge I still have no clue, Smart Keeper mode should be working the same rather you are in a normal run playing as Normal/Tainted Keeper as it would in the challenge, I feel like there might be something more here than I am aware of with the challenge.
TL,DR: "Smart Keeper" mode is to aid with healing, if you don't like it there are multiple ways to bypass it's effects, either temporary or permanent.
