regarding your second statement, im not really sure why that happens, but do know that the generic lord is the faction leader and is not set to immortal, but i honestly dont know why that would happen

One minor oddity I experienced which I don't think is intentional is my initial generic lord disappeared after I replaced him with Petr von Stolpe rather than remaining in my roster as a lord I could recruit for free later, as is usually the case when you use the "Replace Lord" function.
On the other hand, now you know that this mod is working fine, something else may be breaking the scripts

I'm running The Empire Reloaded (https://psteamcommunity.yuanyoumao.com/sharedfiles/filedetails/?id=1863989476) as well as Xoudad's empire-based mods, do you think that could pose an issue?
I'm suspecting that it's probably a Mod conflict issue, what types of mods would this likely be incompatible with?
