Having further tested this I now have an answer to the question.
Everyone needs to have the mod to be able to join a game with the mod.
I suggest adding this to the mod description, could help others wondering.

Now that you mention it I'm pretty sure they were not running the moddable version
That is something we have to test that to see if it makes a difference.

Ah, too bad. Were they running moddable version of the game, though?
Every player has to run moddable to play together, Vanilla client can't join Moddable server.

No, you get a version mismatch.
Was playing with some friends and they got a version mismatch when I was running the mod and they were not.

Found a small problem. When using this mod the Combine Harvester music does not play.
Can you look into this ?

Subscribe to it, launch "moddable" version of the game from Steam, create/join a multiplayer session with 16 players limit.

Nice work! I haven't found any servers to test it, but this feature this was sorely missed from the SS4 launch release. I hope to actually try it out someday. Thanks for adding another piece of the puzzle to make the game whole again.
