would love to do that if the create a new ruler option even worked

1. just create a female ruler first,
2. Then imstead of starting new game, create a Male ruller
(can be done in opposite way as well xD)
3. After that you find the Spouse-to-be with character finder and invite to your court.

Not the same, with this mod you could change, remove or add family with one click, the game will create those with their adn

Mod is Not needed.
You can already make a wifu. Just edit a character female how you want her, then make your character right after. If done right, she should be wandering in the area. Just pick her up before she leaves and gets grabbed by someone else.
