Pls fix icons, as it said below, they are just white squares.

The Vanilla skull you cannot craft, You will have to find it so you are able to craft the modded skull.
Where can you find a skull? Somewhere in the wilderness of Rivellon.

In my game, the Nature-infused skull's icon is just a white square, much like how @bugaboo1234 said it was with the 2h sword..

not sure if your updating this but it seems some of the icons for the 2h sword are broken. they show up as white pictures

only one missing is a necromancer based one. (might be mixed with summon since they have a few summons available to them, so kinda similar to arcane gear set but necro and summon reversed) these have all been excellent! (im using them all on a single playthrough, each armor for set for the respective caster type) :D
