I ran Chilled Cavern and The Hive on Nightmare with this. Both are definitely five skullable without mods (I ran a Blade Staff Sorcress on those levels). I made par time on both, didn't make five skulls- mistimed my use of the Rift Trinket, a few enemies got through. Still lots of fun. It's an appreciable jump in challenge without being silly like 10x enemies.
