Amazing!
How about adding two (jiggleboned) braided cords that hang down from both sides, below the ears?
Most of the woolen knit caps and bobblehats that I see here in Germany in the winter have such cords, to tie the longer side parts below the chin when it's windy in winter. And if you let them hang freely, they look decorative.

It would also make it look as if Medic has two braided twin pigtails. ;-)
Suggestions:
1) Make the opening around the Medic's eye region a bit wider, please. Not much, just so that his expressive eyebrows and eyes can be seen better. Make the rim end just above the eyebrows.
2) Resize the pompom on top, make it a bit smaller (if possible)? Right now it dominates the whole hat. It's the same size as the one on the Heavy's toque, but the Heavy's head is larger than the Medic's.
3) Make the hat longer in the back, so that it covers the back of the Medic's neck? Unless you kept it short on purpose so that it wouldn't clip with shawl miscs like the Mercs Muffler?
