Possible addon crashing. Such glitches were reported several times before and most of them were fixed by unsubscribing the other suspicious addons and restarting their games/PCs.

I dont know what is Entity:GetName()'s return value, maybe you should use something else to find out, if is the spawned NPC the your one or not...
