1. You need to google for a batch file to run which converts everything to the right formats due to engine differences. Depending on where you find it there should be instructions.
2. Make sure you have the recent version of GCF Scape and try again.
3. No idea.
Make the shoulder bones in the same place as the up most spine. That way, if you wanted to reskin the doll to be white and use the color script to diversify, you can do limb transplants!

I have from experience seen that if they are not compiled (or saved in terms of materials) to those specific directories, you will be unable to upload to the workshop.
As for your vmt, you need to put "$texturegroup" "The ages of Cave"}

If you are editing a model and you are just editing the textures then idk why you would need to fix the mdl. I could be wrong.
