I've played around with this mod a lot and found a reliable way to use it. Assuming you have it installed correctly, and there are no conflicts with other mods, I left the details on how it works in a discussion:

I've found surgeon, tried both options, he just follows the character who spoke to him and doing nothing more, no matter what squads I have and wherever I bring him.
Idea is nice but have no clue how to make it work
But as we have no alternative for 5 years, only way is just try try try or give up.. (:

@Nalumah the Shinobi surgeons seem to only have that option sometimes I think I had to go in and out of hub like 3 times untill the line to rename my Garru appeared

This mod is fucking confusing and the description is utterly useless, how does this shit work. The surgeons don't have any different dialogue options, even following these guides.

The instructions are beyond my limited comprehension. Where do you find these Shinobi Surgeons?

seems like can only work 1 time on 1 animal. after tried renaming animal, trigger the animal stat screen, can't do 2nd time as no more such option

The first one has no choice, but the second one exists.
