you can disable it by unsubscribing. If that doesn't work, you can go to the Steam folder in your Program Files (x86) and delete the workshop folder. Or you could go into the workshop folder and attempt to match the size of the mod with the one of the files in the folder, and delete that specific mod.

Does it have to be a model, though? Couldn't it be an actual costume? Disabling these models is a pain as the mod menu doesn't work properly. You can't just disable mods like that.
