When I try to use this it says it has to update and I have to re-add all the items and spells to get them working. How do I do that?

The monster tokens appear to be missing from this.

You need to click on the Workshop link on the top of the right menu. This will open your workshop compendium on the left where you will find 5Extended Monsters and 5Extended Spells & Items. You simply drag and drop from there.
