The only addon that change death sound is this.

First off all you need disable other addons that change death sound and then restart Garry's Mod

It works only 1 time.
When you turn on Garry's Mod again, it stops working and the sound is "This is not the end".
Disabling Slender doesn't help you.
We need help from creator.
