That's because this ONLY works with Afterbirth+

I actually in love with this mod, I’ve been having a lot of fun with it, thanks for making it! I love both DDLC and Isaac so it’s perfect!

The mod is currently getting a complete overhaul. And this version will rollback and remain the Afterbirth+ version.

The new Repentance overhaul will be separate on it's own page

Custom characters don't have completion marks by default. There is however a mod which enables it.

As for character/item being unlockable. I've not looked into how that's possible.
