Actually there was one addon and 3 commands so more like 75% correct

Update: you are actually only halfway correct about it being a conflict with an addon
The main conflict was with Marmotte's First Person Death Mod addon which adds sounds
(I should've read the description but i'm just gonna tell myself I probably would've forgot that detail by now)
I resolved the issue with the console command "fpdm_sounds 0"

also jesus it took me 33 minutes just to turn all them back on and test them and that was with turning multiple addons on at a time to speed it up

Disabled all of them and restarted garry's mod. the addon functions perfectly so it seems you are correct about it being a conflict with another addon. time to find out which one :/

didn't find anything. there were no addons that outright said they added deathsounds and I tried disabling addons that might mess with deathsounds (Now that's what I call dodging affect / effect!)
Looks like I'm going to have to go all in and disable every other addon, and then reenable them one by one to figure out which one(s) are causing the problem.

GMod doesn't have any deathsounds by default. So it's probably conflicting with another addon that adds deathsounds.

For some reason it plays the desired death sound over what would usually play
Example: Death sound is set to metrocop but male rebel and metrocop end up playing at the same time.
I'm guessing it's a problem with installation, and if that's the case, any idea how to fix it?

I'm in a bit of a pickle about testing stuff right now, not sure when I'll have the time to do anything.

Thanks for the report! You're right, they changed all .wav voiceovers to .mp3s. Very random and inconvenient, but should be easy enough to fix. Hopefully I won't break anything in the process since I can't test the changes.

After looking through the tf2_sound_vo_english.vpk, i saw that the files were actually .mp3 instead of .wav, so vo\soldier_painsevere05.wav was actually vo\soldier_painsevere05.mp3
I dont know if thats the reason why the sounds wouldn't play, but its just my guess :)

That usually means TF2 content isn't mounted in GMod (it's glitchy). That, or they changed something in the latest TF2 update. I'll review it whenever I'm free.

I dont know if this has been asked already, but the TF2 human voices dont work. I get these errors in console when selecting each one:
Failed to load sound "vo\soldier_painsevere05.wav", file probably missing from disk/repository
