As Pixenal said below, Medic's head is attached to his body (obviously) while Pyro's head is a seperate smd in the model file. Valve would need to change Medic's model to have his head as a bodygroup.

As the head is the same model as the rest of the body it can not be simply removed, however you could edit this to work for pyro instead, as, unlike the rest of the classes, pyros head is seperate, and therefore can be removed.
