Click on the "props" folder itself to see it, you should.
And if not, make sure you are subscribed to this addon and don't have it disabled.

Or simply mess with it until you figure it out lol
As I wrote in the description, to use it effectively, you need a submaterial tool
As far as I know, none of them are able to fit the texture in 2 ways, as seen on the 4th screenshot of this page. This means that with something like a RT camera, you'd be limited to watching 1:1 image being stretched to 16:9(or even 4:3, that is stretching too) screen. This makes it a bit tedious for those scenery building people who don't feel like using greenscreens and just want a screen with RT camera footage that looks good. Also, even if it was made, as I wrote previously, it was more or less a test for myself if I'd be able to create a neat addon from scratch.
Yet I'm still writing it.
The biggest reason this model exists there is that I wanted to make sure that I will be able to model, texture, compile and upload a model for Source engine from scratch. And, even if someone else made it, it turned out to be a good practice, as next time I will not be wasting 7 hours with different compiling and other problems. Also, even if one exists, people still like it! lol

I suggest you to look up a guide on how to spawn models from Workshop, I'm currently on a phone and it's a bit annoying to type and navigate around.
Or, simply hold Q, in first page(Props) scroll down, open "Addons" folder, and find "Screen model" tab, and look in it.
