Fixed issue with friends not spawning from !friend command when using Afterbirth+

* Removed the top left status pixel (was green if connected, red if not) - most people didn't even notice it, but once you knew, you couldn't unsee it. Has been irrelevant for a long time, so now it's gone.
* Fixed a compatibility issue that was impacting Breath of Life not charging properly.
* Fixed a compatibility issue that was impacting double click items like GD Bug, Hemotypsis, etc
* Due to the above fix, the mod will no longer be able to offer functionality to invert controls etc -- no one was using this functionality from the custom command editor anyhow.
