Fixed selecting off-screen players from commander mode in build 262

- Made all selectables permanently relevant to commanders. Increases network transmission for commander players, but allows hotkey double-tap on everything and reduces the pop-in delay following a minimap click.
- Chat messages are now offset in commander mode so that they don't overlap with player icons so much.

- Remove the default hotgroup commander UI element (player buttons replace it)
