In the train station after going upstairs and reading the note containing the code, head back towards the stairs. At the very top is a chair with a brochure on it.
Note: This only appears in the chair after you read the code note and will not be present prior to this.

Just after you leave the diner and begin to head down the road you will find a phonebox on the left side of the road. Ensure you cross over towards it or it won't ring and may be missed.

This is located inside an open coffin near the end of the graveyard.

As you near the end of the caves, just before coming to the electrified water, you will find a small lantern lit area with the bottle in plain sight.

After passing the lighthouse and moving through the cornfield you will reach a split path. Take the right path towards the farmhouse and move up onto the veranda where you will find the picture on a chair by the door.

After descending the cliff path into the docks area and moving along the boardwalk a short way you will find a phonebox on the right side of the path.

After passing by the raised bridge into the forest you will pass a set of gates with animal skulls tied to them. Continue on a short while and you will see a burning barrel with an animal skull to the left of it.
Just being in the proximity of the skull will trigger the voiceover. Interaction is not possible/required.

Just down the road from Old Libby/Witch's house you will find a phonebox once again on the left side of the road.

As you approach the hospital, head right into the carpark and the voiceover will trigger as you near some of the cars.

Once you take the first elevator up, head to the end of the room into the small office/reception area. The phone on the desk will ring.

Once you take the elevator up to the third floor, turn right and head down the long corridor to the end. The final door on the left will be accessible and a phone will ring on the desk.

As you are heading down a long corridor in the morgue, this voiceover will trigger. It cannot be missed.

After finding the empty chamber in the morgue and returning to the previous room, the phone on a gurney will begin to ring.
