By default all of the variants have 20% chance to appear in place of corresponding enemy.
(TEMPORARILY DISABLED) You can easily adjust spawn chances or disable some variants in config.lua file in the mod's directory (Documents/My Games/The Binding of Isaac Afterbirth+ Mods/enemyvariantpack_925168037)

If the API will be improved (Antibirth team is working on Repentance so I hope it will) then I will certainly make some cool stuff with the alt path.

When the Repentance dlc comes out would it be cool to per say make Creepy Boom Fly which i feel could be renamed to just Creepy spawn in Dross which Downpour's alt in Repentance and same goes with the Pestilence Head and make i dunno the Devil Nightcrawler which i feel would be renamed to Devilcralwer spawn in Sheol and Dark Room and Crying Host put into Downpour or Flooded Caves you know.

It works for me even after redownloading from workshop. Are you sure this is not an issue on your end?
