All the textures (except default Source Engine textures) are also custom made from scratch in Photoshop.
Nothing is stolen from anywhere else.

Just keep in mind that the models were designed specifically for the map, so they might not have proper physics or animations when spawned as a prop. The chomp was designed to just follow a set path, so didn't need physics
most likely it's something like gmr_blockfort_vX... the in-game console should autocomplete it for you if you start typing one of the map-related commands, like "changelevel gmr_....."
If you want to use the model outside of Garry's Mod, you could try using a map unpacker to extract the model
