I'm not sure if I want to release a different version of this asset, but if so, I'll announce it here.
This can happen because the distance between underground tunnels is really low and when the ground is not flat, the tunnels get skewed and as a result might collide, which causes certain road pieces not to be built. To fix this, I would have to put in more space between tunnels, which would increase the footprint (and cost) of this intersection.
But there is an easy solution: Use the new landscaping tools that were introduced with patch 1.4 to flatten the terrain , and you'll be fine :)
Are you sure you have Network Extensions Project installed? The interchange won't work without it (it relies on the two-lange highways which aren't available in vanilla).
If that doesn't solve the issue, can you post a screenshot?
