United States addon (16.3+)

Enables NASL and USL.

This is only compatible with the 16.3 database (known as the 'Official Winter Update').

Known issues:

* Puerto Rico FC doesn't generate any new players. There's nothing I can do about this. Players that move to Puerto Rico FC become eligible for the Puerto Rico national team after spending five years after the age of 18 at the club.
* Austin Aztex are in the USL. This is deliberate as I couldn't get the USL working with 29 teams.
