No bots, two players co-op. The amount of zombies, tank and witch HP are decreased twice. Only one SI at a time.

Didnt work with 2 players, server crash everytime

Just checked, wasn't killing Spitter and nothing else spawned except Tank. I think Tank and Witch don't count in the limits, but other SI do.
May be you have this issue on some specific map?

Not sure why, but we're still getting multiple SI at a time when we play.
Tried through a dedicated im hosting, as well as a local server.

Crash should be fixed. It wasn't a mutation fault but I found a workaround on the Source wiki.
Challenge fully works, just tested.
