This is a good adaptation of the single player map. The doors closing and hurting everyone still left outside when the rocket launches is a good feature and I wanted to make a map like this, but just taking the single player map and adapting it was a good call and makes my idea seem silly in comparison.

It's alphabetical order, but sometimes it gets rearranged in favor of the VFS order (i.e. which vpk, directory, gcf the file is in, etc)

Once subscribed and downloaded it should appear in the "Create Server" dialog box accessed from the main menu (the maps in that dialog aren't always sorted in alphabetical order, so it might be somewhere near the top or bottom)
