L4D1-Style Death Screen

After finding the unused L4D2 death screen it inspired me to frankenstein both the L4D1 and L4D2 death screens together to make whats shown here. Between the games they changed internally how the health display works on spectator, so I wasn't able to get the scoreboard-like healthbars or the dead local player. I did get the tip text though!!!

Will conflict with anything that modifies spectatorsurvivor.res
