In order to install this add-on, you'll need to download two programs:

• Navigate to the Add-ons directory: \Steam\steamapps\common\Left 4 Dead 2\left4dead2\addons\workshop.
  
• Find 3323050906.vpk and open with GCFScape.
  
• Drag and drop the transition folder into \Steam\steamapps\common\Left 4 Dead 2 (alongside left4dead2.exe).

• Navigate to the left4dead2 directory: \Steam\steamapps\common\Left 4 Dead 2\left4dead2.
  
• Find gameinfo.txt and make a backup copy.

• Open gameinfo.txt with Notepad++.
  
• Find SearchPaths. It will look something like this:

Game update
Game left4dead2_dlc3
Game left4dead2_dlc2
Game left4dead2_dlc1
Game |gameinfo_path|.
Game hl2
}

• Add a new line above update for the transition folder so it looks like this:

SearchPaths
{
Game transition
Game update
Game left4dead2_dlc3
Game left4dead2_dlc2
Game left4dead2_dlc1
Game |gameinfo_path|.
Game hl2
}

• Be sure to save the changes and you're done!

This is essentially forcing the game to load this folder when it first launches.

This was designed to go very well alongside Qude's Clean Transition, so the two borders at the top and bottom that would normally be present have not been modified.
