I actually did a little more testing with the mod and it seems it has to do with length.
Longer titles seem to often end up on your smaller line with your name. More often it is the suffix that ends up there also.

That title is manually given out by dan, as for the titles ill take a look, its possible that some titles were still missing in the list, ill take a look at it. Language shouldnt matter unless the titles are being translated to a language other than english.

Btw in French (idk if that matters) I have Guardian and Omnipresent, there is no third line it just puts Guardian first and big and second line is my name + the omnipresent in small letters
