Multiple game sessions on the Very Low setting and Insane Rare Loot did not provide a fun gameplay experience, so I was trying to tune it so that it's a bit more realistic and give you time to play and find food, antibiotics or a way to disinfect.
Because even if you change the Dirty Bandages for some reason you could randomly catch an infection and sometimes your life keeps ticking down even when not infected and not dirty bandaged.
Anyway, this version is half the damage of the Very Low setting in the original mod. And 9 "One Minute Ticks" more for a wound to get infected from a Dirty Bandage. Just trying stuff out until it feels right.
Anyway, since you're playing on 4 hour long days the original mod might be fine for you, makes sense. With 1 hour long days everything goes 4x faster if you think about it.
In "Normal' mode, you lose about 1 hit point every 10 game minutes when wearing a dirty bandage on a bleeding and infected wound. Meaining with a 4 hour long game day, you can see your hit points dropping visually and die in around 1 game hour.
In "low" it takes about 3 game hours.
Plenty of time to find new bandages, clean one's and food to stay fed and healthy.
How does this one differ?
