These hearts have a small chance to replace other heart types. When they are picked up, they have a random number of pips on them, ranging from 1 pip to 5. When Isaac's dice heart gets depleted, it's special effect actives. The effect depends on the number of pips on the heart.

The only form of health Isaac'D can have is dice hearts. He cannot have any other form of health. Through his pocket active, he is able to "reshuffle" his dice hearts, changing the amount of pips that they have.

When playing as Isaac'D all hearts have a 50% chance to turn into a dice heart and a 50% chance to morph into another pickup. When Isaac'D is accompanied by another character, the hearts do not morph into another pickup and rather stay as they are.
