As the name says, here is a simple guide for the achievements.
This is not a walkthrough and, except the screenshots, has no spoilers.

A rare picture

In the level 1, in the room where you find the note #8, interact with the picture on the chair. Story related, this achievement is unmissable.

Level 1 gold figurines & notes

This level includes 6 golden figurines and 9 notes.

Note #1

In the second room, read the note pinned on the right beam. Hard to miss.

Gold figurine #1

In the third room, climb the chair against the bookshelves then grab the small horse figurine above, near the ceiling. Easy to miss.

Note #2

Further in the same room, read the note on the round table. Hard to miss.

Note #3

Next room, on the ground near a small table with pots. Hard to miss.

Note #4

In the room opened with the 1st key, on the round table with red candle. Hard to miss.

Gold figurine #2

In the same room, near the table with note #4, grab the small the small horse figurine in the small dresser at the back. Medium to miss.

Note #5

On the round table, at the corner of the long corridor after you found the 2 parts of the woman statue bust. Hard to miss.

Gold figurine #3

As soon as you enter the room full of shelves covered with blankets, look at the ground on your left, between two shelves. Easy to miss.

Note #6

After the door opened with the 2nd key, in the room with upside down table and soil spilled everywhere on the ground, the note is on the table with two plants. Hard to miss.

Note #7

Further in the level, you'll be in a study room with 3 statues and 2 tables with red candles. The note is on a table with big jar. Note: you also can find the 3rd key in this room. Hard to miss.

Gold figurine #4

In the small area with red valve, opened with the 3rd key, on the ground. Hard to miss.

Note #8

Further in the level, you enter a large room with fireplace and a picture on a chair. The note is on the round table at the back of the room. Hard to miss.

Note #9

Just after the note #8, you can notice another note on the ground, at the angle of the corridor, in front of an ajar door. QUICKLY go read this note before a rat grabs it away. Easy to miss.

Gold figurine #5

Just behind the door ajar, near the note #9, Hard to miss.

Gold figurine #6

In the labyrinth with shelves with barrels full of red wine (or blood?), instead of turning left into a long corridor, go straight on then look on the ground to the left. Easy to miss.

Level 2 gold figurines & notes

This level includes 5 notes and 4 gold figurines.

Gold figurine #1

As soon as you start, turn back then walk around the crates by the right. Easy to miss.

Note #1

A bit further in front of you as you start the level, on a plank. Really hard to miss.

Gold figurine #2

At the start, go to very end of the corridor. The statue is behind a wooden grate. Hard to miss.

Note #2

After the first flooded part, the note is on a table near a sitting corpse, in the infirmary. Hard to miss.

Gold figurine #3

Immediately after the crawl section in water, turn right and search for the figurine near the rubble. Medium to miss.

Gold figurine #4

Look at under the big ugly monster. You have to grab the gold figurine quickly before you get killed by the monster's jumping. (thanks to the dev RedMiller for the hint) Easy to miss.

Note #3

A bit further the little puzzle involving a water bucket, you can find the note on a lit table. Hard to miss.

Note #4

Just after the corridor full of statues, you'll find the note on a table near a fireplace, in a small archive room. Hard to miss.

Note #5

A little bit after the note #4, next to a skeleton. Hard to miss.

Notes #2 achievement unlocked!

Level 3 gold figurines & notes

This level includes 4 notes and only 2 golden figurines.

Note #1

By the first house, next to the skeleton. Hard to miss.

Note #2

Behind the previous house, enter the half buried house by crawling. The note is at the back on the left. Hard to miss.

Gold figurine #1

To the opposite side of the note #2. Hard to miss.

Note #3 & Gold figurine #2

After you repaired the carriage and got in the house, look immediately on the right. The note is on the round table and the gold figurine on the shelves next to the table. Hard to miss.

Note #4

Downstairs, on a table to the left, under a big clock. Hard to miss.

Notes #3 achievement unlocked!

Level 4 gold figurines & notes

This level includes 7 notes and 5 gold figurines.

Note #1

At the beginning, go right at the first junction. You will find the note on a table, at the end of the corridor. Hard to miss.

Note #2

Further in the big corridor, ignore the door with missing mask and take the first right corridor. The note is next to a skeleton, at the back of the bedroom. Note: don't forget the key in the fireplace. Hard to miss.

Gold figurine #1

From the note #2, go back to the main corridor and take right then to the end. The gold figurine is on the small table under the window. Hard to miss.

Gold figurine #2

After opened the door with the key, you have to get the mask from the statue. Instead of taking the mask from the statue, go to the end of the corridor. The gold statue is on the ground in the darkness, near the scaffoldings. Easy to miss.

Gold figurine #3

Go to the end of the corridor with the bell. The gold figurine is on the ground. Medium to miss.

Note #3

When you wake up in the tunnel, follow the only way to the note. Really hard to miss.

Note #4

After the 4 busts puzzle, you'll come to a T-shaped junction with a candle on the ground. Take a left and the note is in the next room, on a table. Medium to miss.

Note #5

A long way after the previous note, you'll come in a large room that looks like a barn. The note is upstairs, on a pillar. Hard to miss.

Note #6

After the running challenge in the huuuuuuge barn, read the note on the teddy bear. Hard to miss.

Note #7

In the crates maze, follow the only way to the note and skeleton in the small dead end. Hard to miss.

Notes #4 achievement unlocked!

Gold figurine #4

From the note #7, take back and follow the only way, then immediately turn left. The gold figurine is on the ground next to a candle. Easy to miss.

Gold figurine #5

From the gold figurine #3, take back and follow the only way again. A bit further, you'll crawl under a statue's arm. The gold figurine is in the large area just after the statue. Medium to miss.

Level 5 gold figurines & notes

This level includes only 1 note and only 1 gold figurine.

The last note of the game is in front of you as you start the level. Really hard to miss...

Gold figurine

Just after the note, turn left in the corridor and crawl under the upside down shelves. The last gold figurine of the game is at the end. Easy to miss...
