Counter Strike: Source is required and the original Freddy's map.

(i will take this down with the creators request. to regain power, take the crowbar and put the crowbar on the door in the employees only room. name of map is "freddysold" to get the link to this map, right click on the subscribe button and press "copy page url.")

you can do whatever you want to this map, i mean it isn't mine so ports and other stuff is fine by me

So, a commenter by the name of krpwilliamson recently figured out a fix for the addon.
Thanks to him (or her) for figuring it out!

I have missing texture. i dowloaded map from workshop dowloader because i couldn't find it in files.

gmod can't detect the bsp from the workshop some reason, you have to manually copy the file from "steamapps/workshop/4000/2640944612" and paste it in "GarrysMod/garrysmod/maps" (i copy and paste it in so i don't have to reinstall
