I do run an absurd amount of mods, more than 40 I believe. Yours was pretty far down the list since I figured it didn't conflict with anything else I was running. However, I don't recall the name of the event, but it gave my Norse heidinn emperor the nickname "the Great", and gave me the emperor title of "None", effectively striping me of my current. It was also reaccuring (after I had restored my title). It happened to character A three times, and once to his son (after which I took the reference to"the Akan" at heart and disabled the mod ;)).
Perhaps you can add some triggers to the event? Religion, culture and/or perhaps having certain provinces?
