This is one of the limitations of the way this mod is implemented (you can see a full list of limitations on the main Xbox HUD page). The Y-button, dpad, and boxes around the dpad aren't really there, they're basically just textures that are anchored to your health bar. So whenever your health bar is on screen, those UI elements will be too, even when the rest of the HUD is supposed to disappear.

I noticed these stay on the screen whenever the gui disappears, like when you're grabbed by a Hunter, Smoker or even get sent into the air by a Charger's charge.

Noticed steam adds an Icon when using controller and they appear when using this mod. Could there be an update or a new one that over ride the steam one?
