@OneSpookyBoi eldercare is free with the patch, that came along the Sunset Harbor release, so you don't need to buy the Sunset Harbor expansion for this asset to work, but eldercare should be available from the moment your game has been updated. Eldercare buildings is located under the "Health" menu.

@pensfan044 interesting mod. If the mod gets an update i might make a version for that, but i would highly prefer it updated, because it seems like some players are having issues with it.
