This is Nerub, a Ruin only mini boss you can find in the sewers.
You access the sewers in any level of difficulty and fight some rats, but Nerub will only show himself in champion difficulty.
You’ll be rewarded with four new trinkets of the Vermine set.

  • Not compatible with any mod that modifies \dungeons\crypts\crypts.props.darkest

heads up mod conflict with nightmare mod sewer crashes game if you interact with it but not game breaking

Dude these trinkets suck so much, tails is just worse legendary bracer that hurts your team, heart is worse debuff amulet with stress, the stake is cool but it should have corpse clear always, and the claw is not strong enough to make up for disabling friendly skills.
Boss mods NEED good rewards to be something you actively want to do.
