From the mod text...
To use the Really Advance Setup mod in your game first enable it then click "Next".
Then click "Single Player" then "Set Up Game" then "Advanced Setup".
Do not click "Back". That just causes you to exit the mod.
Are you clicking on the check mark next to my mod so that it turns green?
If you still need help. You could try verifying the game files in steam. For instructions try searching for "verifying the game files in steam".
If that doesn't work. You could try clearing the Civ 5 cache. See my post below for instructions.
You could try clearing the Civ5 cache.
Make sure you exit Civ 5.
Find the Civ 5 cache folder. Its location might vary slightly depending on what operating system you have. Mine is located at: "Documents\My Games\Sid Meier's Civilization 5\cache".
Delete the cache folder.
Restart Civ 5 in the usual fashion. Civ 5 will automatically recreate the cache when the game starts.
You will have to re-enable any mods that you wish to use. I recommend you just use this mod by itself for the first test after clearing the cache. Then add other mods later.
If that doesn't work. You could try deleting this mod's saved data -> "Documents\My Games\Sid Meier's Civilization 5\ModUserData\GTAS_AdvancedSetupMod-15.db". Delete the file while the game is not running. Then the next time you play the game with this mod it will set everything back to their default values.
If you are talking about the number of players in a map. That is dependent on map type and map size. So, it varies depending on the settings and there is no single default.
You can add players using the "Add AI player" button at the bottom of the panel.
You can remove a player by clicking on the small x in the upper left corner of their panel.
The number of civs and city states are set back to their default level when you change the map type or size.
As a test try changing to a different map. Preferably one of the standard ones that came with the game.
This mod doesn't support multi-player. As best as I can tell it's not possible due to the way the mod works.
