I was truly awestruck by the sheer beauty of this game upon having the pleasure of playing it for the first time. You are plunged into a maddening world, with only your rod by your side. The ending of this game was truly a twist of the century, and personally had me weeping for multiple hours after its completion. The creator of the notorious Demonophobia/Xenophobia returns after almost two decades, to bring you this... mess of a game. Pretty late to the party here, but now things have hopefully died down.

"Dysthanasia is the unethical, slow, and painful prolongation of a patients life through unnecessary medical intervention, often referred to as "cruel death." It focuses on maintaining life without regard to the patients suffering or quality of life."

You wake up in an unknown facility without any memories, and have to sneak your way around morbid entities that either want to kill, or use you as an onahole. Its a very basic stealth horror, with a handful of puzzles. You may find items or props on the way, to make life easier, you wont unlock anything in the gallery if you dont put yourself in harms way however. Theres a good amount of sections and enemies to traverse. The map design is generally blocky and crude, it feels very amateurish and raw, but thats somewhat fitting. Theres unexpectedly even an interesting story attached to it, albeit, only told through notes, centering around experiments concerning rebirth and immortality.
The adult content is nothing short of wild, and extremely high quality, they didnt leave out any details. Being split in half, staked, turned into soup cubes, acid, flesh eating walls. Probably the most detailed 3D guro game to date. Its generally either Visual Novels or sidescrollers.

Theres a lot of bad and/or obvious oversights, which leaves one puzzled how that made it through. The MCs head is crudely attached to her body, you can see how badly it is stitched together under her chin. Even with the patch she has no private parts whatsoever, but the original demo did. Some of the sex scenes has the creatures ♥♥♥♥ protruding out of her back. Breaks immersion left and right. Some of the later models look rushed, especially the 4 armed guy, like they were 3D printed or AI generated, didnt fit the vibe with the others.
The gallery consists of pre-rendered videos, and some suffer from weird depth of field, resulting in scenes becoming blurred and unfocused. The wall crush death is literally just a giant blurry video. Turning DOF off in the settings obviously does nothing. I have no idea if this was an oversight or an actual design choice. They never even bothered updating anything in these 5 months, aside from some FPS issues. Wild.

Engaging with the game has not been very fun in the past, the publishers actively removed and banned screenshots from people, even if it didnt show any nudity. Reviews got deleted, because they mention the developers history. I wonder if its even worth wasting my time on this review. Madowaku is understandably ultra paranoid, because their games even got banned on DLSite, and these mofos allow EVERYTHING. Having to go through such stuff and being "hunted" for making controversial games sure is debilitating. I feel sorry for them, but at the same time, this aint the way. Stand by what you are and your creation, or just dont bother at all, your only hurting your actual fans.

If they cared to fix aformentioned stuff it would be a definitive recommend. As a gamer I say no, as a guro fan I say yes. But hey, I only got an empty star review, so my words are worthless anyway!
