You were aiming just slightly high and the Mag-7 has spread, so it has a number of pellets (8 at 30 dmg each) and with each shot those pellets spread out. Here you just got some unfortunate spread, if you look at the frame right after your shot, you can see that 5 pellets missed his head, so you only hit him with 3 of them, and of those probably not all of them went into the head.

If you look at the frame where you kill the 2nd guy, your crosshair is actually slightly below and to the left of his head, so here you got some fortunate spread working for you.

Because of the spread, it's sometimes better to aim slightly below the head with a Mag7, or at least on the lower half of the head.
