I completed the game and did not obtain it. I assume it's either for starting the game or for falling in the elevator shaft.

You said you restarted the game from scratch, but we still wonder if the achievements on the actual game will be fixed and if you can tell us what to do to get each of them because no one completed the game yet and for many of us, completion is important. A reply would be nice....
